Home Care Wellness ("we," "us," or "our") operates homecarewellness.net and provides the Unify Wellness Plan to home care agencies and their employees. This policy explains what information we collect, how we use it, and the choices you have.
Information we collect
Information you give us directly. When you fill out a form on our site, we collect your name, business name, legal business name, email address, phone number, and any other information you choose to provide.
Payroll information you upload. To prepare a savings analysis, we ask you to upload a payroll report. We ask that you remove employee names, Social Security numbers, home addresses, and any protected health information before uploading. We only need wage amounts and the number of employees. Uploaded files are transmitted over an encrypted connection and stored in access-restricted systems.
Information collected automatically. When you visit our site we may collect your IP address, browser type, device type, pages viewed, and referring URL through cookies and similar technologies.

Text messaging and mobile information
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All of the categories described in this policy exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
If you provide your mobile number and consent to receive text messages, we may send you messages related to your inquiry, appointment reminders, and information about the Unify Wellness Plan.
Message and data rates may apply. Message frequency varies. Consent to receive text messages is not a condition of purchase or of receiving a savings analysis.
You may opt out at any time by replying STOP to any message. You will receive a single confirmation that you have been unsubscribed, after which we will send no further marketing texts. Reply HELP for assistance, or contact us at the phone number or email below.
Email communications
We send email about your inquiry and about our services. Every marketing email includes an unsubscribe link. You can also reply to any message and ask us to stop, and we will honor that request.

We share information only as needed to deliver our services:
Service providers who help us operate, such as our CRM, email delivery, scheduling, and cloud storage vendors. These providers are bound by contract to protect your information and use it only to provide services to us.
Plan partners. If you request a savings analysis or enroll, we share the information necessary to prepare your analysis and administer the plan with UnifyWell and our licensed benefits partners.
Legal requirements. When required by law, subpoena, or to protect our rights or the safety of others.
We do not sell your personal information. We do not share your mobile number or SMS consent with anyone for their own marketing.
Data retention and security
We keep information for as long as needed to provide our services and to meet legal and recordkeeping obligations. We use industry-standard safeguards including encryption in transit and access controls. No method of transmission or storage is completely secure, so we cannot guarantee absolute security.
Your choices and rights
You may request a copy of the personal information we hold about you, ask us to correct it, or ask us to delete it. Our services are for businesses and are not directed to anyone under 18. We do not knowingly collect information from children.
