Caregiver Burnout: What Home Care Agencies Can Actually Offer
Median direct care pay was $17.36 an hour in 2024. Burnout guides written for agencies stop at scheduling, matching and recognition, which are the levers that cost nothing and reach only the hours a caregiver is on the clock. The rest is money. A Section 125 wellness plan reaches that part without new payroll. How far it reaches depends on your roster.
Search "caregiver burnout home care agency" and read the first ten results.
They recommend better scheduling, client matching, recognition and check-ins. All of it is sound. All of it is also free advice about the eight hours a caregiver spends at work, and burnout does not stay at work when the shift ends.
The other sixteen hours are where the unfilled prescription sits, along with the tooth ignored since March and the counseling appointment somebody priced once and never booked. Nobody writing those articles is wrong. They are writing about the part an agency can reach without money, which is the only part most agencies can reach.
What is actually wearing them down
You already priced this and it did not work
You costed a raise. Two dollars an hour across forty caregivers runs $134,360 a year once employer FICA is added, which on a seven to ten percent net margin is the whole margin.
A Section 125 wellness plan is funded differently. It does not ask for new money. It redirects employer payroll tax the agency already sends out the door into benefits the caregiver can use, which is why net employer cost lands near zero rather than at $134,360. It is the same logic under retention without raising pay.
Most of my caregivers are part-time
Thirty-hour eligibility is a group carrier rule. It is not tax law. A Section 125 election is open to W-2 employees regardless of hours, so a caregiver working twelve hours one week and thirty the next is still eligible to participate.

What that looks like in one household
Denise picks up about three days a week and splits them between two agencies. Her mother moved in last spring, her hours have not been steady since, and the counseling she looked into once ran about $140 a session out of pocket. She priced it and never booked it. Under the wellness plan that counselor is a phone call at nine at night, with no copay and no waiting room. The generic she takes for blood pressure costs her nothing at the pharmacy she already uses.

Where this does not help
Benefits do not fix a broken schedule. If clients are matched badly and shifts move with no notice, no benefit holds anyone in the job. The plan also returns less for a caregiver working very few hours, because the whole design runs on redirecting payroll tax and there is less of it there to redirect.
What would this look like at your agency?
Two numbers answer it. How many caregivers you run, and roughly what they earn. That is the whole ask, and it stays small on purpose, because nobody should have to hand over paperwork just to learn whether something is worth a conversation.
The estimator takes those two numbers and shows what the agency currently pays in employer payroll tax, what turnover at the industry median costs it, and what could be redirected into caregiver benefits instead. Thirty seconds, no signup.
UnifyWell's published example nets an employer roughly $640 per participating employee per year on a $44,000 earner, after the monthly administration fee. Caregiver wages sit below that. Across Home Care Wellness proposals the figure has landed between $300 and $900 per participating employee per year, which is a range and not a quote. Only your own numbers answer which caregivers qualify and what lands on their check.

Home Care Wellness is not a law firm, accounting firm or insurance agency, and nothing here is legal, tax or insurance advice. A Section 125 wellness plan is not health insurance. Figures shown are estimates that vary by wage mix, participation and state, and no result is guaranteed. Reductions in workers compensation premium may also occur but depend on the carrier and the state. Plan compliance opinions referenced here were issued by CBIZ and HitesmanLaw.
