What we provide

We provide information about the Unify Wellness Plan and, at your request, prepare a no-cost savings analysis based on payroll information you supply. Submitting a form does not create any obligation for you to enroll, and does not obligate us to provide services.

Savings estimates are estimates

Any savings figures we provide are estimates based on the payroll data you give us and on current tax law and plan design. Your actual results will depend on your employee census, participation rates, wage levels, applicable law, and other factors. Estimates are not a guarantee of results, and nothing on this site is a promise of any specific financial outcome.

Not tax, legal, or financial advice

We are not your accountant, attorney, or financial advisor. Information provided by us, including savings analyses and plan proposals, is for general

informational purposes and does not constitute tax, legal, accounting, or investment advice. You should consult your own qualified professionals before making decisions about employee benefits, payroll, or tax treatment.

Information you provide

You agree that any information you submit is accurate and that you have the authority to submit it. When uploading payroll files, you agree to remove employee names, Social Security numbers, home addresses, and any protected health information before uploading. We only need wage amounts and employee counts. You are responsible for what you upload.

Communications

By submitting a form, you agree that we may contact you by email and phone about your inquiry. If you separately check the box consenting to text messages, you agree to receive SMS from us at the number you provided, including messages sent using automated technology.

Message and data rates may apply. Message frequency varies. Consent to receive text messages is not a condition of purchase or of receiving a savings analysis. Reply STOP to opt out at any time, or HELP for assistance. See our Privacy Policy for how we handle your information.

Acceptable use

You agree not to use this site to upload malicious code, attempt to gain unauthorized access, scrape or harvest data, interfere with the operation of the site, or use it for any unlawful purpose.

Intellectual property

All content on this site, including text, graphics, logos, and materials, is owned by Home Care Wellness or its licensors and is protected by applicable intellectual property laws. You may not reproduce or distribute it without our written permission.

Third parties

The Unify Wellness Plan is administered in partnership with UnifyWell and licensed benefits professionals. Their services are governed by their own agreements and terms. We are not responsible for the content of third-party websites linked from this site.

Disclaimer of warranties

This site and its content are provided "as is" and "as available" without warranties of any kind, express or implied, including any implied warranties of merchantability, fitness for a particular purpose, or non-infringement. We do not warrant that the site will be uninterrupted or error free.
